Background: The current influenza epidemic threshold in Korea is defined as the average plus two standard deviations of ILI data during non-epidemic periods over the past three years. However, this approach does not adequately exclude the influence of non-influenza respiratory viruses, often leading to an overestimation of the epidemic period. Since the transition of COVID-19 to endemic status, a lack of objective and quantitative thresholds has hindered timely public health response and medical resource allocation. Purpose of study: This study aims to develop an improved method for defining influenza epidemic thresholds and to explore the feasibility of establishing COVID-19 thresholds using Korea’s sentinel surveillance data. Method ‐ We collected and integrated influenza surveillance data from KDCA and healthcare claims data from HIRA (2014–2024) to conduct time-series analyses. ‐ Multiple methodological approaches were applied and compared, including composite indicators (ILI and positivity rate), the Moving Epidemic Method (MEM), change point detection, and standard deviation-based thresholds. ‐ International practices and trends in respiratory virus surveillance from the U.S. CDC, ECDC, and Japan were reviewed. Results ‐ For influenza, the MEM-derived thresholds closely aligned with epidemic patterns observed in health claims data. However, considering the complexity of implementation, a more practical alternative was identified: combining ILI thresholds with a minimum 5% influenza positivity rate. This approach enhances specificity while maintaining continuity with the existing surveillance system. ‐ In contrast, defining a fixed threshold for COVID-19 remains statistically unreliable due to insufficient historical data, irregular epidemic waves, and weak seasonality. Conclusion ‐ It is recommended that influenza epidemic thresholds be revised to require both an ILI exceedance and a positivity rate of at least 5%. ‐ For COVID-19, instead of static numerical thresholds, a multi-indicator monitoring framework—tracking hospitalization rates, viral mutations, and environmental signals—should be adopted. ‐ In the long term, integrating all sentinel systems into a unified respiratory virus surveillance platform is essential for sustainable epidemic monitoring.
