Background and Objectives: Since 2015, the number of reported Q fever cases in South Korea has been consistently occurring. While many patients recover from acute Q fever, a subset progresses to chronic Q fever, a condition associated with high fatality rates. Despite this, there is a complete lack of research on the incidence, risk factors, and clinical characteristics of chronic Q fever in Korea. Therefore, this study was conducted to investigate the current status of chronic Q fever in Korea and to establish a Korean-specific prevention and management strategy. Methods: This study secured evidence through a multi-faceted approach. 1) Big Data Analysis: Analyzed the prevalence and risk factors of chronic Q fever using data from the Health Insurance Review and Assessment Service (HIRA) (2015–2024). 2) Multi-center Clinical Cohort: Conducted an in-depth analysis of medical records and epidemiological investigation reports for 410 Q fever patients from 51 medical institutions nationwide. 3) Awareness Survey: Conducted a survey targeting 156 medical personnel and public health officers responsible for infectious disease managemen. Results - Estimated incidence of Chronic Q Fever: Analysis of HIRA data revealed that among Q fever patients, 14.8% were identified as 'Possible chronic Q fever' cases suspected of having chronic complications, and 8.3% were 'Probable chronic Q fever' cases requiring actual pharmacological treatment. Based on estimates from the HIRA claims data, chronic Q fever in Korea accounts for at least 6.1% and up to 14.8% of all Q fever cases. This proportion is likely to be heavily influenced by whether mild or asymptomatic Q fever cases are diagnosed and by how actively clinicians suspect and investigate chronic Q fever in patients with vascular infection, endocarditis, or spondylitis. - Clinical Characteristics of Korean Chronic Q Fever: The Netherlands and France where endocarditis or vascular infections are predominant, the most common manifestation in Korean chronic Q fever patients was osteomyelitis and spondylitis (49.2%). This was followed by endocarditis (25.0%) and vascular infections (23.9%). - High-Risk Groups and Risk Factors: Independent risk factors for the development of chronic Q fever were identified as: Age 65 or older, Vascular disease, Valvular heart disease, Immunocompromised state ㅇConclusion: 1) Development of Korean Clinical Guidelines: Guidelines should be revised to reflect the 'predominance of bone and joint infections' distinctive to Korea. Differential diagnosis for Q fever should be recommended for patients with osteomyelitis or spondylitis of unknown origin. 2) One Health Surveillance System. 2) To block the transmission link between livestock and humans, joint epidemiological investigations with animal quarantine agencies should be strengthened. 3) Introduction of a Targeted Management System for High-Risk Groups: High-risk groups—such as patients with valvular heart disease, artificial vascular grafts/joints, and immunocompromised individuals—must be categorized separately
