This study is an extension of the [Korean HBV Cohort Study], leveraging its infrastructure to register not only treated patients previously collected but also immune-tolerant or inactive HBeAg(-) chronic hepatitis B patients who could potentially transition to treatment in the future. The primary goal is to facilitate long-term follow-up and blood sampling to establish a robust research infrastructure for future studies. Although previous research suggests that immune cell compositions within the liver environment are not well-reflected in the blood for hepatitis B, the study aims to accumulate resources that could be utilized with advancements in research techniques. Over three years, 315 patients were recruited, including 205 immune-tolerant or inactive HBeAg(-) hepatitis B patients targeted for blood sampling. From 146 patients, 206 blood samples (an average of 1.4 per patient) were collected as part of the study. The research protocol, which involves vertical blood sampling 1-2 times yearly, was confirmed to operate smoothly. However, the short three-year duration of the study was insufficient to secure a large-scale cohort. Using data from this study and the existing [Korean Hepatitis B Cohort Study], clinical analysis revealed that immune-tolerant and HBeAg(-) inactive hepatitis B patients with elevated baseline ALT, increased AFP (a tumor marker) and Log10 HBV DNA levels of 4–6 IU/mL exhibited independently higher activation frequencies. However, this finding is limited by unexamined variables, and validation through multi-center data analysis beyond the scope of this study is necessary for generalization. Additional analyses using the stored biospecimens are also anticipated. In conclusion, this study establishes a foundation for identifying activation factors in the under-researched populations of immune-tolerant and HBeAg(-) inactive hepatitis B patients. It successfully identified predictive clinical indicators for activation using clinical parameters.
