제3차 국가 항생제 내성 관리 대책(2026~2030) 수립을 위한 전략 및 지표 개발 연구

초록

1. 제2기(2021–2025) 국가 항생제 내성 관리대책의 평가 및 시사점 제2기 대책은 One Health 기반의 관리전략을 본격화하며, 항생제 적정사용, 내성균 관리, 감시체계 강화, 연구개발 및 협력체계 고도화를 주요 목표로 추진되었음. ASP시범사업, KONAS 구축, Kor-GLASS 확장 등 정량적 성과는 달성하였으나, 중소·요양병원 및 의원급 의료기관으로의 확산 부진, 부처 간 데이터 연계 미흡, 감시체계의 정책 환류 기능 부족이 한계로 확인되었음. 비인체 분야에서도 농축수산물 전자처방제, 잔류 항생제 관리체계가 일부 진전되었으나, One Health 차원의 통합거버넌스 부재가 지속적으로 지적됨. 이에 따라 향후 대책에서는 성과 중심의 관리체계, 통합데이터 기반의 정책 피드백, 중소병원 중심의 내성관리 역량 강화가 핵심 과제로 제시됨 2. 제3기(2026–2030) 국가 항생제 내성 관리대책의 전략 및 제언 (1) 항생제 적정사용 정착 및 확산: 의료기관 규모별 맞춤형 ASP 수행지침을 마련하고, 전문인력 양성·교육·수가체계를 연계한 지속 가능한 기반을 구축해야 함. 질적평가 주기화, 외래 급성감염 항생제 미처방 인센티브 도입, AI 기반 처방모니터링 시스템 구축이 필요함. (2) 내성균 관리체계 고도화: 감염관리 전문인력의 자격·보상체계를 제도화하고, 지역 거점병원을 중심으로 한 자문 네트워크(ICCON 등) 확대가 필요함. AI·로보틱스 기반 감염예방관리 자동화, 환자 전원 시 내성균 정보공유 시스템 구축을 통해 효율성을 높여야 함. (3) 감시체계의 통합 및 디지털 전환: 기존 KONAS·Kor-GLASS를 연계하여 AU–AR–임상결과 간 통합분석이 가능한 플랫폼을 마련하고, AI 기반 위험예측·조기경보 시스템을 구축함으로써 실시간 환류체계를 실현해야 함. 비인체(축·수산·환경) 감시체계도 통합하여 One Health 기반의 다부처 공동감시체계를 완성해야 함. (4) 거버넌스 및 제도적 기반 강화: 항생제 적정사용 관련 법제화를 추진하고, 중앙–지방–민간 협력 네트워크를 제도화하며, 성과지표 기반 인센티브 체계를 도입해야 함. 범부처 협의체를 중심으로 인체·비인체 연계, 국제공조, 예산 연동형 정책 거버넌스 구축이 요구됨.

1. Evaluation and Implications of the Second National AMR Control Plan (2021–2025) The second plan advanced a One Health-based management strategy, focusing on appropriate antibiotic use, resistant organism control, surveillance enhancement, research and development, and intersectoral collaboration. Notable achievements included the nationwide ASP pilot project, the establishment of the KONAS surveillance system, and the expansion of Kor-GLASS. However, several challenges remained—limited dissemination of ASP activities to small and long-term care hospitals, insufficient inter-ministerial data integration, and inadequate policy feedback mechanisms within existing surveillance systems. In the non-human sector, partial progress was made in electronic prescription and residue monitoring systems for livestock and aquaculture, but a comprehensive One Health governance framework has yet to be established. Accordingly, the next phase must prioritize a performance-driven management framework, integrated data utilization for policy feedback, and strengthened AMR control capacity in community and long-term care settings. 2. Strategies and Recommendations for the Third National AMR Control Plan (2026–2030) (1) Institutionalizing and Expanding Antimicrobial Stewardship (ASP): Develop tailored ASP implementation guidelines for different hospital levels and ensure sustainable infrastructure by linking workforce training, education, and reimbursement systems. Introduce regular qualitative evaluations, outpatient incentives for non-prescription in acute infections, and AI-driven prescription monitoring. (2) Enhancing AMR Control Systems: Institutionalize qualification and compensation standards for infection control professionals and expand regional consultation networks (e.g., ICCON). Adopt AI and robotics-based automation in infection prevention and establish inter-hospital AMR data-sharing systems to improve efficiency and response. (3) Integrating and Digitizing Surveillance Systems: Link existing surveillance programs (KONAS, Kor-GLASS) into an integrated platform enabling AU–AR–clinical outcome analysis. Implement AI-based predictive analytics and early-warning systems for real-time feedback. Integrate non-human (livestock, aquaculture, environment) data to establish a One Health multisectoral surveillance framework. (4) Strengthening Governance and Institutional Foundations: Pursue legal institutionalization of antimicrobial stewardship, formalize central–local–private collaborative networks, and introduce performance-based incentive mechanisms. Under an inter-ministerial governance structure, strengthen human–animal–environmental linkages, international collaboration, and budget-linked policy coordination.
