This study analyzed the prevalence, metabolic and lifestyle characteristics, medication treatment, comorbidity patterns, and mortality risk prediction models for elderly diabetes patients in South Korea. The goal was to provide a multidimensional evidence base for managing diabetes in the elderly. The prevalence of diabetes and prediabetes in elderly individuals aged 65 and older was found to be significantly high. This is due to rapid aging, unique metabolic and lifestyle characteristics in Korea, and socio-environmental changes, with a rising number of newly diagnosed diabetes patients, signaling future public health challenges. On the positive side, awareness, treatment, and glycemic control rates (HbA1c <7.5%) were above 70%, reflecting the improvements in Korea's healthcare system, better accessibility, and increased health literacy. However, challenges remain, including low physical activity levels (around 40%) and high rates of obesity and abdominal obesity, indicating that simply increasing physical activity is insufficient to address metabolic imbalances. The increasing prevalence of age-related diseases, such as dementia, heart failure, and chronic kidney disease, has made managing these conditions more complex. End-stage kidney disease was found to be the leading contributor to increased mortality risk, with dementia, cancer, and heart failure also playing significant roles. In terms of medication treatment, elderly patients had similar medication use patterns as the general population, but adherence was lower in those starting insulin therapy or with severe comorbidities, due to factors like injection complexity, cognitive decline, and social isolation. This highlights the need for multifaceted interventions, including specialized nurse education, expanded family support, and comprehensive monitoring. The study also developed a health risk score model using the Health Insurance Corporation's database to predict mortality risk in approximately 870,000 elderly diabetes patients. The model showed that ultra-high-risk groups had more than 30 times the mortality risk compared to low-risk groups, with excellent predictive ability (c-index = 0.781), providing evidence for early identification of high-risk patients and more efficient allocation of healthcare resources. In conclusion, while the high prevalence of diabetes in the elderly presents challenges, improvements in Korea's healthcare system have helped achieve favorable awareness, treatment, and glycemic control rates. The study emphasizes the need for personalized management strategies that reflect the complexity of the elderly population, integrated management of multiple diseases, and the development of high-risk screening models. Future efforts should focus on high-quality cohort studies, interdisciplinary healthcare systems, and expanded patient-centered education to improve the quality of life and extend the healthy lifespan of elderly diabetes patients.
