❍ This study aimed to integrate fragmented infection prevention and control (IPC) evaluation schemes—Infection Control Status Survey, KONIS, Appropriateness Evaluation of Health Care Benefits, Healthcare Quality Assessment, and Healthcare Accreditation—into a KDCA-led national IPC evaluation governance framework, to reduce evaluation blind spots in smalland medium-sized hospitals and long-term care hospitals and to guide linkage of IPC evaluation with incentives and reimbursement. ❍ Major domestic IPC-related schemes were compared by purpose, legal basis, evaluation methods, and indicator sets; indicator overlap and key limitations were assessed; and international IPC evaluation/surveillance systems linked to payment were reviewed. Multidisciplinary expert consultations were conducted to derive integrated governance options, directions for legal and fee-schedule reform, a phased 2026–2031 roadmap, and strategies to promote participation. ❍ The analysis showed a dual structure—surveillance/investigation versus evaluation/support—with fragmented agencies and data pathways. Structure and process indicators were widely duplicated, whereas outcome indicators were limited. In the United States and Europe, outcome indicators (e.g., surgical site infections, device-associated infections, antimicrobial use) are routinely used and linked to financial support; however, despite preparatory research in Korea, no routine national system currently collects and operationally uses such outcomes. ❍ Four governance alternatives were proposed: a direct implementation model with a dedicated KDCA unit (Ⅰ-1), an evaluation-dedicated upgrade of the status survey into a national IPC status evaluation including outcome indicators (Ⅰ-2), a contracted model entrusting evaluation to a third-party evaluation agency and linking results to the HIRA payment/incentive system (Ⅰ-3), and a distributed model allocating functions across HIRA and the accreditation body (Ⅱ). Options were compared by legal and fiscal requirements, feasibility, linkage strength, and expected support effects for smaller facilities. ❍ The study recommended redefining the healthcare facility IPC status survey as a healthcare facility IPC evaluation and embedding indicator standardization and mutual recognition, sustainable financing, and tailored support for smaller facilities within legal and payment frameworks. A phased 2026–2031 roadmap (initial–mid-term–scale-up) was proposed to establish a dedicated unit and indicator system, expand participation, and institutionalize nationwide outcome-based performance management through an evaluation–incentives–support–feedback cycle complemented by non-financial incentives.
