[Background/Objectives] While Surgical Site Infections (SSIs) pose a significant threat to patient safety, current national policies, such as the 'Operating Room Patient Safety Management Fee,' are primarily designed for large hospitals. This leaves small to medium-sized hospitals and clinics in a policy blind spot with limited infrastructure and resources. This study aimed to develop an evidence-based 'Operating Room Infection Control Consultation Tool' tailored to these institutions, verify its effectiveness through pilot operations, and establish an educational foundation for sustainable capacity building. [Methods] A preliminary set of items was derived through a review of domestic and international guidelines (WHO, CDC, AORN, etc.). The final tool, consisting of 100 items across 6 domains, was established through a two-round Delphi survey involving both infection control experts and field practitioners. Pilot operations were conducted in 9 medical institutions to analyze implementation rates. Additionally, an educational needs analysis was performed using Borich needs assessment and the Locus for Focus model targeting operating room staff. [Results] The pilot operation revealed a significant disparity: the high-compliance group achieved an implementation rate of 91.5%, whereas the vulnerable group (7 hospitals) showed severe deficiencies in 'Operating Systems' (29.6%) and 'Medical Device Reprocessing' (32.6%). Critical issues, such as the absence of sterile storage rooms and the lack of separation between clean and contaminated zones, were identified as "structural impossibilities" due to physical space limitations rather than mere negligence. The educational needs analysis highlighted an urgent demand for training in practical topics such as 'ventilator circuit management,' 'establishment of regulations,' and 'sterilization verification (BI/CI).' [Conclusion and Discussion] This study is significant as it presents the first infection control standard specialized for small to medium-sized medical institutions in Korea. The results indicate that substantial support beyond the simple distribution of the tool is essential. Therefore, this report recommends the creation of a "Support-type Infection Control Fee" to aid infrastructure improvement, the introduction of a "Blended Learning Program" combining e-learning and on-site training, and the legalization of realistic facility standards. The developed consultation tool serves not as a regulatory instrument but as a "navigation system" for self-diagnosis and improvement, thereby contributing to the standardization and elevation of the national patient safety net.
