Introduction: Chickenpox is an acute infectious disease caused by the varicella-zoster virus (VZV). It was introduced to the National Immunization Program in 2005 and is administered once to infants aged 12-15 months. Despite maintaining a vaccination rate of over 97%, chickenpox cases continue to be reported. This study aimed to analyze the current status of chickenpox outbreaks and the effectiveness of the vaccination program in Korea to establish an appropriate vaccination strategy. This report presents the final results by summarizing four years of research. Methods: Children and adolescents born between 2011 and 2023 and registered in the national immunization registry were included in this study. We investigated varicella vaccination history and the occurrence of reported chickenpox cases, then constructed a cohort and designed a case-control study within the cohort. In parallel, we also constructed a cohort using health insurance claim data to investigate the occurrence of chickenpox. In the case-control study, we compared the characteristics of chickenpox patients and controls using the chi-square test and t-test. A 1:1 matched-pairs study was conducted using conditional logistic regression analysis to evaluate vaccine effectiveness and time to onset after vaccination. Unconditional logistic regression analysis was used for the 1:1 matched case-control study. In the cohort analysis using the Health Insurance Review & Assessment Service's healthcare big data system, we followed up on children born between 2011 and 2023 to compare the differences in vaccine effectiveness by vaccination status, number of doses, follow-up period, and vaccine type. Kaplan-Meier survival curves and Cox proportional-hazard modeling were used to calculate hazard ratios for chickenpox development and estimate vaccine effectiveness. Results: The effectiveness of two doses of the varicella vaccine was higher and maintained over the long term compared to one dose. This suggests that a single dose of the varicella vaccine may not provide sufficient immunity, and that two doses can - 8 - enhance the prevention of varicella infection. However, vaccines introduced after 2018 have not had a sufficient follow-up period to evaluate their effectiveness. Additionally, the overlap with the COVID-19 pandemic may have led to an overestimation of vaccine effectiveness. Therefore, further research through long-term follow-up is necessary to accurately assess the effectiveness of these vaccines. The peak incidence of varicella after vaccination was observed at age 6 in both the one-dose and two-dose groups. The recommended ages for varicella vaccination are 12-15 months for the first dose and 4-6 years for the second dose (based on U.S. guidelines). According to this study, the peak incidence of varicella after vaccination occurred within 5 years in the one-dose group and within 2 years in the two-dose group. This means that even after vaccination, it is possible to contract varicella before the age of 6, after the recommended vaccination ages. This implies that various factors including individual differences in immunity and virus mutations can influence varicella incidence after vaccination Conclusion: In parallel conducted case-control and retrospective cohort studies, involving the entire birth cohort of children and adolescents in Korea, using data from the national immunization registry, national infectious disease surveillance system, and health insurance records, demonstrated that two doses of the varicella vaccine were more effective and maintained long-term protection compared to one dose. Particularly for vaccines introduced since 2018, the observation period was before the onset of waning immunity, and the impact of COVID-19 may have resulted in an insufficient evaluation of vaccine effectiveness. Therefore, prospective long-term follow-up is needed to more accurately assess the vaccine effectiveness.
