The purpose of this study is to calculate the medical expenses of tuberculosis (TB) patients in Korea and to assess the level of economic burden resulting from these costs. Specifically, the study aims to: first, determine the incidence rate of catastrophic costs incurred by households of TB patients in Korea; and second, identify the causes and mechanisms leading to catastrophic costs due to TB. This study was designed as a cross-sectional investigation employing a mixed methods approach that included both surveys and in-depth interviews. The main findings of the study are as follows: First, the survey results indicate that approximately 4 to 5 out of every 10 TB patients in Korea face catastrophic costs due to TB. The primary source of these costs is not medical expenses but rather non-medical costs—including expenses for nutritional supplements and other necessary items—and indirect costs resulting from income loss. Second, the in-depth interview results revealed a multidimensional process in the occurrence of catastrophic costs. Although “treatment costs” may be covered by government policies, the direct non-medical costs incurred during the treatment process—which include the time and monetary costs for hospital transportation as well as the expenses associated with hiring replacement labor for hospitalization or outpatient care—remain unprotected. Moreover, even when family members or acquaintances provide unpaid caregiving, such labor can also be considered a cost. The process of income losswas identified as involving job loss, voluntary resignation, and the hiring of replacement labor. Based on the study’s findings, the following policy recommendations are proposed: First, the institutionalization of catastrophic cost monitoring and further research for additional policy development should be pursued. Second, as a TB-sensitive social protection measure, the implementation of paid sick leave and disease allowance is necessary. Third, the introduction of TB-specific social protection programs is required.
