This study aims to systematically collect data on rare diseases and establish a national database to support the production of official statistics. The goal is to provide an evidence base for policy-making in areas such as research, diagnosis, and the development of treatments for rare diseases. The revision of the rare disease registration guidelines was conducted through a comprehensive review of domestic and international literature, as well as a detailed examination of medical records of patients with rare diseases at university hospitals. Based on these findings, clear and detailed guidelines were developed. • Definitions for each registration item were standardized in explanatory Form, with clearly articulated input criteria and specific examples to ensure user comprehension and practical application. In particular, the criteria for entering disability type and severity were aligned with relevant legislation. • Sources of information for each registration item (e.g., medical records) were identified, and concrete examples were provided for key fields such as the date of first visit and date of initial diagnosis. • Methods for confirming final diagnoses of rare diseases were categorized into imaging studies, special biochemical/immunologic tests, smear/culture tests, genetic testing, histological examinations, and clinical diagnoses. For each method, the conceptual basis and disease-specific features were illustrated with applied examples based on clinical knowledge. • Given that over 80% of rare diseases are genetic or congenital in nature and closely related to genetic mutations, the purposes and interpretations of tests such as Karyotyping, FISH, MLPA, CMA, Sanger sequencing, Panels, WES, and WGS were explained using representative cases of rare diseases with distinctive genetic variants. Training materials for rare disease registration practitioners were developed with reference to existing data collection projects (e.g., cancer registry and inpatient injury surveys) and through consultation with clinical experts. Major diseases were selected, and real-world cases were used to create practical, case-based materials. • Lupus nephritis and Moyamoya disease were chosen as the target diseases for training material development. Using actual inpatient cases from university hospitals, the materials were designed to facilitate clear understanding of data sources (e.g., medical records) for fields such as the date of first visit, date of initial diagnosis, diagnostic method, and interpretation of results. This study contributes to enhancing the accuracy of the 2025 rare disease survey results and improving the credibility of national statistics by laying the foundation for standardized guidelines and practical training of registration personnel. Going forward, to further strengthen the foundation for rare disease research, it will be essential to expand and refine registration items, develop a comprehensive casebook, and establish robust procedures and methodologies for data quality management. In addition, tailored training materials for registration personnel should be developed and progressively expanded.
