ㅇ 수집된 균주 대상 emm 유형 분자유전학적 분석에서는 emm1(20.8%)이 가장 흔했으며, 그 외 emm12(17.0%), emm28(11.3%), emm3(9.4%) 순으로 분포하였다.

In South Korea, Scarlet fever is designated as a notifiable infectious disease and is subject to nationwide surveillance. However, there is no surveillance system for invasive group A streptococcal (iGAS) infections, making it difficult to systematically assess the incidence and mortality associated with these infections. Therefore, this study examined the surveillance systems for iGAS infections in countries, confirming that the United States, United Kingdom, Canada, Australia, Spain, and Japan operate such systems. Most countries implement comprehensive surveillance, while some states in the United States conduct clinical and laboratory surveillance through population-based sentinel monitoring systems. Countries with surveillance systems have been able to quickly identify changes in the incidence and outbreak patterns of iGAS infections and respond with policy measures. Notably, after the COVID-19 pandemic, both infection cases and mortality rates increased. In response, the United Kingdom expanded its pediatric and adolescent pharyngitis treatment guidelines to include adults. In contrast, South Korea has comprehensive surveillance for Scarlet fever but lacks a systematic approach to monitoring iGAS infections, highlighting the need to expand surveillance for tracking infections and analyzing circulating strains. To address this, this study established (1) an expert network (including specialists in pediatric infectious diseases, infectious disease medicine, laboratory medicine, and preventive medicine), (2) a nationwide multi-center surveillance system, and (3) developed case report forms for iGAS infections. (4) retrospective and prospective surveillance systems were piloted from 2015 to 2024, (5) an epidemiological investigation form for iGAS infections was developed, incorporating expert opinions collected through surveys, and (6) expert surveys were conducted on the necessity of a national surveillance system for iGAS infections. Strain collection and molecular genetic analysis were also conducted. Over the past 10 years (2015–2024), a total of 383 cases of iGAS infection were reported. Among them, 319 cases (83.3%) occurred in adults, while 64 cases (16.7%) were in children. Clinical risk factors were identified in 60.8% (n=233) of cases, and 52.2% (n=200) had potential infection routes, social or epidemiological risk factors. Regarding treatment, 159 cases (41.5%) required surgery or skin incision procedures, while 5 cases (1.3%) underwent amputation. Intensive care unit admission was necessary for 27.2% of patients. The overall mortality rate was 14.4%, and 11.7% of patients experienced long-term complications. emm typing analysis showed that emm1 (20.8%) was the most common, followed by emm12 (17.0%), emm28 (11.3%), and emm3 (9.4%). Analysis of strains collected from 2023 to 2024 after the COVID-19 pandemic did not reveal a predominant genetic type; however, emm1 was consistently detected. Additionally, two isolates (collected in 2020 and 2024) were identified as the M1UK strain, which has increased virulence. According to expert surveys, 85.4% of respondents agreed on the need for continued surveillance and preventive measures for Scarlet fever. Additionally, 70.7% supported comprehensive surveillance for iGAS infections and streptococcal toxic shock syndrome. Many respondents emphasized the necessity of laboratory-based surveillance. This study confirms the necessity of establishing a surveillance system for iGAS infections. Such a system would enable monitoring of epidemiological changes in South Korea, manage high-risk groups, and early outbreak detection, ultimately contributing to improved patient care and increased awareness among healthcare professionals and the public.
