호남권역 지역 특성 및 감염병 발생 분석에 따른 대응 전략 제시 연구

초록

ㅇ연구목적 호남권 지역에서 발생하는 감염병을 이해하고, 발생 감염병에 대한 선제적 예방 관리전략을 마련하기 위하여 수행됨. 이를 달성하기 위한 연구의 구체적 목표는 첫째, 호남권 지역 특성과 감염병 발생 양상을 분석, 둘째, 선제적 감염병 예방관리 방안 및 효율적 관리전략의 제시, 셋째, 호남권 지역에서 발생한 감염병의 발생 연보를 작성하는 것임. ㅇ연구방법 - 호남권 감염병 발생 양상을 확인하기 위하여 호남권역, 4개 광역시도 및 44개 시군구를 대상으로 전수신고 대상 감염병의 2001년부터 2023년까지의 발생 현황과 시계열적 추세분석을 실시하였음. 또한, 감염병 발생 관련요인을 확인하기 위하여 9개 영역의 지역 특성을 확인하고, 질병 발생의 공간상관성을 검토한 후 적절한 회귀모형을 선정하여 관련요인을 확인하였음. - 한국, WHO, 미국, 영국, 호주 및 일본의 감염병 관리 정책을 검토하였고, 주요 국가의 감염병 관리 정책을 법적 기반, 관리 체계, 예방접종 정책, 주요 감염병 대응 전략, 데이터와 기술 활용, 국제 협력 등의 측면에서 종합적으로 검토함. - 감염병의 예방 및 관리에 관한 법률에 근거하여 국가 감염병 감시체계를 통해 질병관리청에 신고·보고된 자료를 기초로 호남권 지역의 통합된 발생 감시 연보를 제작함. ㅇ연구결과 - 호남권의 법정감염병은 전국 발생 수준보다 높은 수준을 유지하고 있으며, 호남권에서 높은 발생률을 보이는 감염병은 결핵, 수두, 장출혈성대장균감염증, 성홍열, B형간염, C형간염, 비브리오패혈증, 쯔쯔가무시증, 렙토스피라증, 신증후군출혈열, 중증혈소판감소증후군 등임. - 호남권에서 증가 추세에 있는 감염병은 수두, 장출혈성대장균감염증, 파라티푸스, 유행성이하선염, 폐렴구군감염증, 성홍열, B형간염, 쯔쯔가무시증, 신증후군출혈열, 크로이츠펠트-야콥병 및 변종 크로이츠펠트-야곱병과 중증열성혈소판감소증후군 등이었으며, 감소 추세를 보인 감염병은 결핵, 세균성이질, C형간염, 말라리아, 비브리오패혈증, 렙토스피라증 등임. - 호남권의 감염병 발생과 관련이 있는 지역 특성은 인간의 감염에 대한 감수성, 기후와 날씨, 변화하는 생태계, 인구학적 특성 및 건강행태, 공중보건조치의 붕괴 및 빈곤과 사회적 불평등 영역이었음. - 외국의 정책과 비교하여 우리나라 정책의 취약점으로는 예방접종 및 국민인식 영역에서 감염병 관련 교육 및 홍보 부족, 통합 데이터 체계 영역에서 국제 협력 데이터 공유가 제한점임, 디지털 플랫폼 구축과 관련하여 예방접종기록의 디지털화 노력 부족, 감염병 예측 및 경고 디지털 시스템 미비 등을 들 수 있음. ㅇ고찰 및 제언 호남권 지역의 선제적 감염병 예방관리를 위하여 첫째, 감염병 대응 역량 강화, 둘째, 감염병 감시체계 개선 및 강화, 셋째, 예방접종률 향상을 위한 제고 및 효과 확인, 넷째, 매개생물 및 인수공통감염병 관리 강화, 다섯째, 권역 및 지역의 감염병 치료 및 진단 역량 강화를 제언함.

ㅇ Research Objectives This study was conducted to understand infectious diseases occurring in the Honam region and to develop a preemptive preventive management strategy for outbreaks of infectious diseases. ㅇ Methods 1) The characteristics of the Honam region and the pattern of infectious disease outbreaks analyzed; Temporal trend analysis, spatial regression model 2) Preemptive infectious disease prevention and management measures and efficient management strategies were presented 3) The bulletin of infectious diseases occurring in the Honam region was created. ㅇ Results - The incidence of infectious diseases in Honam region remains higher than the national level, especially tuberculosis, chickenpox, enterohemorrhagic E. coli infection, hepatitis(B & C), vibrio septicemia, tsutsugamushi, leptospirosis, nephrotic syndrome hemorrhagic fever, and severe fever with thrombocytopenia syndrome. - Infectious diseases with an increasing trend in the Honam region include chickenpox, enterohemorrhagic E. coli infection, mumps, hepatitis B, tsutsugamushi, hemorrhagic fever with nephrotic syndrome and SFTS, while infectious diseases with a decreasing trend include tuberculosis, bacterial dysentery, hepatitis C, vibrio septicemia, and leptospirosis. - Regional characteristics associated with infectious disease outbreaks in the Honam region include human susceptibility to infection, climate and weather, changing ecosystems, demographic characteristics and health behaviors, breakdowns in public health measures, and poverty and social inequality. - Weaknesses of Korean policies compared to those of other countries include lack of education and publicity related to infectious diseases in the areas of vaccination and public awareness, lack of international cooperation data sharing in the area of integrated data system, lack of efforts to digitize vaccination records in relation to the establishment of a digital platform, and lack of a digital system for infectious disease prediction and warning. ㅇ Discussion In order to proactively prevent and manage infectious diseases in the Honam region, first, strengthening infectious disease response capacity, second, improving and strengthening the infectious disease surveillance system, third, improving and confirming the effectiveness of increasing vaccination rates, fourth, strengthening management of vector-borne and zoonotic infectious diseases, and fifth, strengthening regional and local infectious disease treatment and diagnosis capacity are suggested.
