This study aimed to quantitatively evaluate the validity and reliability of surveillance data from the neonatal intensive care unit (NICU) healthcare-associated infection surveillance system (KONIS-NICU) and to propose directions for improving its future operation. From July 2021 to June 2024, external validation, internal validation, and denominator data validation surveys were conducted across multiple institutions nationwide. For the external validation survey, 21 hospitals were selected from among 80 participating institutions in 2025, considering infection rates, NICU size, and regional distribution. Investigators conducted on-site reviews of at least 40 cases per hospital, totaling 1,005 cases, using blood culture–positive lists and inpatient rosters. Of 162 bloodstream infections (BSIs) registered in KONIS, 143 were concordant with surveyor diagnoses, while 19 were discrepant. Specificity and predictive values were high, but sensitivity was 71.5%, slightly lower than in the previous survey (79.4%). Sensitivity was particularly lower in large NICUs (≥30 beds), hospitals with extremely high or low BSI rates, and non-metropolitan institutions. The main reasons for discrepancies were insufficient understanding of diagnostic criteria, particularly in applying the revised secondary BSI definition and interpreting LCBI2 with clinical symptoms. These findings underscore the need for regular training and case-based education. Surveyor feedback also highlighted heavy workload, with some visits exceeding 8 hours, suggesting the need to limit case numbers (≤40 per hospital) and simplify survey content. Nevertheless, 85% of participating hospitals reported that external validation was helpful in reviewing and improving their own surveillance practices, confirming its practical utility. The internal validation survey was conducted in 26 hospitals, including newly participating institutions and those with no reported BSIs. A comparison of hospital-reported blood culture–positive cases with KONIS data identified 24 diagnostic errors (5.4%) among 445 cases. Errors were concentrated in a few recently joined hospitals, likely reflecting challenges during early adaptation to the surveillance system. Internal validation yielded a sensitivity of 52.4%, which was lower than in 2021 (88.9%), although specificity (99.0%) and predictive values (84.6–95.2%) remained high. An additional 20 unreported BSIs were identified, indicating the risk of underreporting. Less than half of institutions conducted regular internal reviews, and multi-reviewer systems were limited. However, more than 70% of hospitals acknowledged the usefulness of internal validation. For the first time, denominator data validation was attempted, focusing on patient-days and central line-days. While 71% of hospitals collected denominator data electronically, only 33% performed regular accuracy checks. Moreover, some hospitals experienced confusion in interpreting case definitions due to insufficiently clear criteria for device insertion and removal times, highlighting the need for improved standardization. Through these validation surveys, this study identified key areas for improvement, including: (1) reinforcing regular education and case-based training on infection definitions, (2) strengthening initial support for newly participating institutions, and (3) establishing a systematic denominator data verification process. For future KONIS-NICU validation, hospital selection should not rely solely on single indices such as SIR or CAD but instead adopt a multi-layered approach considering BSI incidence, new participation, regional balance, and NICU size. In particular, targeted strategies are needed for large and non-metropolitan hospitals where sensitivity was low. External validation is suggested to conduct at three-year intervals, with case numbers limited to 40 per hospital. Internal validation should be institutionalized through a rotational schedule that ensures participation of all hospitals over time. In conclusion, the KONIS-NICU validation survey is a valuable tool for strengthening the accuracy, reliability, and efficiency of the national NICU surveillance system. The findings and proposed improvements are expected to enhance the development of NICU infection surveillance and ultimately contribute to improving patient safety in neonatal intensive care.
