This study aimed to establish a foundation for the development of bacteriophages for the treatment of multidrug-resistant (MDR) bacterial infections. To achieve this, a research model for bacteriophages was established, research and development strategies were formulated, and bacteriophage resources for therapeutic development were collected and characterized. Experimental protocols for bacteriophage research were established through paper review, attendance at external seminars, and laboratory visits. A total of 15 experimental protocols were developed. Additionally, an advisory committee composed of bacteriophage experts was formed to devise a research strategy for the development of bacteriophages targeting MDR bacteria. Based on the committee’s recommendations, research was conducted, and visits to overseas phage therapy centers provided insights into clinical applications and library management methods. To collect bacteriophages with diverse bactericidal activity for national research resources, bacteriophages that specifically infect MDR E. coli, K. pneumoniae, P. aeruginosa, and A. baumannii were isolated from hospital wastewater. Initially, bacteriophages targeting ESBL-producing E. coli and K. pneumoniae were isolated, and their host specificity was analyzed. However, due to their low lytic activity against carbapenemase-producing strains, additional bacteriophages targeting carbapenem-resistant bacteria were collected. Through studies using clinical isolates, potential candidates effective against various MDR bacteria were selected. Further host specificity analysis of the newly acquired bacteriophages identified candidates with high lytic activity against specific strains. In particular, P. aeruginosa-specific bacteriophages exhibited a broad host range, whereas A. baumannii-specific bacteriophages had a narrower host range, highlighting the need for additional resource collection and screening. Based on these findings, environmental factors and optimal host bacterial selection strategies for securing MDR bacteriophage resources were proposed. Furthermore, whole-genome sequencing techniques for bacteriophages were established to verify safety and analyze mutations that may occur during propagation. While attempts were made to analyze host bacterial determinants, currently available bioinformatics tools allowed only limited analysis, indicating the necessity of database expansion in the future. In conclusion, this study secured 120 bacteriophage strains effective against MDR E. coli, K. pneumoniae, P. aeruginosa, and A. baumannii. The phenotypic and genotypic characteristics of these bacteriophages were analyzed, laying the groundwork for the development of therapeutic bacteriophages. These findings will be utilized for establishing and managing a therapeutic bacteriophage library for national research resources.
