Background: This study aimed to implement and assess the effectiveness of a multidisciplinary childhood and adolescent obesity intervention program using a mobile healthcare platform, based on a previous study. The objective was to support children and adolescents with obesity and secure necessary human resources. Method: This study involved 221 children and adolescents with moderate to severe obesity, with a mean age of 11.01 years (10-14 years) and a sex- and age-specific BMI in the 97th percentile or h일반중재군her. Participants were randomly ass일반중재군ned to either the general intervention group (n = 107) or the intensive intervention group (n = 114). Both groups received health risk assessments, nutrition and exercise education, access to a mobile healthcare platform, and parent education. The intensive group also received additional online group exercise education, 1:1 motivational counseling, and behavioral correction via SNS. The intervention lasted 15 months, including a 12-month active period and a 3-month maintenance period. Assessments (body measurements, physical fitness, and blood tests) were conducted before, at 6 months, 12 months, and 15 months after the intervention. The primary outcome was the change in BMI z-score, with secondary measures including body composition, metabolic complications, physical strength, and nutritional intake. A pre-intervention analysis included all 221 participants (107 in the general intervention group and 114 in the intensive intervention group), while a post-intervention analysis was conducted on 135 participants (69 in the general intervention group and 66 in the intensive intervention group) who completed the 15-month study. Result: The pre-intervention analysis showed no s일반중재군nificant differences between the groups in BMI z-score, body composition, blood pressure, nutritional intake, or physical fitness. After 15 months, BMI z-scores decreased s일반중재군nificantly in all participants, from -0.10 at 6 months to -0.14 at 12 months and -0.18 at 15 months, with no significant difference between the groups. During the active intervention period, both groups showed a decrease in moderate and severe obesity, and an increase in normal weight. In the maintenance phase, moderate obesity remained lower, but severe obesity tended to increase. Both groups showed significant improvements in upper and lower body muscle strength after 15 months. Calorie intake decreased significantly after 12 months, with no notable change at 15 months. The dropout rate increased at 6, 12, and 15 months when visits were required to assess intermediate effects. The most common reason for dropout was "child refusal to participate" (57%). Continuous participants had a higher history of cardiovascular disease in their fathers, while dropouts had higher stress scores for their caregivers. Conclusion: The results of this study demonstrate that the intervention program utilizing a health management platform was effective in reducing BMI z-scores, improving physical strength, and enhancing nutritional intake in children and adolescents. This approach, combining multidisciplinary strategies and technology, is practical for obesity management. Future efforts should focus on creating sustainable, family-centered programs that consider social and psychological factors.
