Iatrogenic Creutzfeldt–Jakob disease (iCJD), although rare, constitutes a high-impact risk because a single event—owing to its long incubation period, nonspecific early symptoms, and the extraordinary resistance of prions to sterilization—can undermine public trust in healthcare institutions and lead to large-scale lookback investigations, device disposal, and substantial legal and administrative costs. While no cases of variant CJD (vCJD) have been reported domestically and most occurrences remain sporadic, potential transmission routes through medical interventions such as dura and corneal transplantation or high-risk neurosurgical procedures still exist. Yet practical limitations in Real-Time Quaking-Induced Conversion (RT-QuIC) testing, the difficulty of obtaining definitive preoperative confirmation in emergency or elderly patients, and the inherent risk of operational gaps mean that the current public-health system cannot fully eliminate iCJD solely through screening and diagnostic measures. Therefore, this project proposes a hybrid strategy that reinforces the frontline of prevention and screening while simultaneously establishing a post-exposure containment mechanism capable of rapidly absorbing and blocking harm when failures or oversights occur. In this framework, preoperative cerebrospinal fluid RT-QuIC testing becomes mandatory only for the highest-priority core group —patients who meet both procedural-risk criteria (brain, spinal cord, or posterior eye segment) and either clinical-risk or exposure-risk factors—and, when preoperative sampling is not feasible, intraoperative collection immediately after dural opening is standardized so that isolation and sterilization intensity can be adjusted promptly based on test results. Independently of clinical symptoms, all procedures involving high-infectivity tissues adopt as a standing standard the use of dedicated instrument sets, single-use devices, or prion-specific sterilization protocols involving chemical pretreatment with 1–2 N NaOH or ≥2% active-chlorine hypochlorite for one hour, followed by steam sterilization at 134°C for at least 18 minutes—preferably 60 minutes when possible—with institutions unable to adopt the full protocol immediately applying at minimum the 134°C for ≥18 minutes standard and upgrading to chemical pretreatment and dedicated or single-use sets as resources allow. Although this results in a 21–213% increase in cost, the additional expense is treated as a predictable premium that transforms a low-frequency catastrophic loss into a manageable routine expenditure. Should exposure nevertheless occur, a time-based “Golden Period” action plan is activated without delay: during the first 0–60 minutes, instruments are kept moist and isolated, tagged to prevent prion fixation, and restricted from movement; within 1–24 hours, prion-specific sterilization or disposal is determined based on WHO and KDCA guidelines, legal notification is completed, and initial exposure classification is finalized; between 24 and 72 hours, high- and moderate-risk individuals are notified and communication is standardized to minimize anxiety; and subsequently, lookback procedures are completed using identifiers such as endoscope serial numbers, set codes, and sterilization labels, followed by device disposal or high-risk reprocessing, patient notification, and a fully documented Root Cause Analysis. Throughout this process, an Instrumental Tracking System (ITS) links instruments, sets, sterilization batches, and patient IDs through barcode, RFID, or UDI technology, enabling precise determination of exposed individuals based on actual usage logs and reducing unnecessary device disposal. To ensure continuous feedback between preventive and post-exposure systems, a National CJD Autopsy Support & Surveillance Program (NCASP) is established to connect a central 24/7 hub with regional partner hospitals, standardized limited-autopsy kits, electronic chain-of-custody procedures, and biobank and quality-assurance infrastructure, while participation incentives such as swing beds, advance reimbursement, and priority scoring promote engagement and ensure that findings are automatically integrated into KDCA guidelines, training programs, and Key Performance Indicators (KPIs). Clear role division further strengthens the system: the KDCA oversees standards, training, KPIs, NCASP, and national coordination; local governments and public health centers handle incident intake, joint risk assessment, communication, follow-up, and reporting; and medical institutions are responsible for rapid reporting, isolation, prion-specific sterilization or disposal, lookback procedures, root-cause analysis, and ITS operation. Core KPIs—including dedicated-set utilization rates, compliance with prion-specific sterilization, completeness of ITS tracking, notification within 24 hours, communication completion within 72 hours, and autopsy-rate improvement from 5% to 15% to 25% over three years—guide performance evaluation. Implementation proceeds in stages, beginning with point-of-use moisture retention, labeling, tagging, and checklist deployment; advancing in the short term to dedicated-set use and prion-specific sterilization algorithms; expanding in the mid-term to full ITS deployment and targeted RT-QuIC normalization; and culminating in the mid- to long-term institutionalization of NCASP, swing-bed mechanisms, and compensation frameworks. Ultimately, by blocking potential transmission through screening, dedicated sets, and prion-specific sterilization, and by absorbing any failures through Golden-Period response, ITS-based precision tracing, and autopsy-driven feedback, the strategy transforms iCJD from an unpredictable catastrophic risk into a manageable and transparent operational process, thereby ensuring both feasibility and public trust.
