The cohort study of uberculosis is a multicenter, prospective observational cohort study that registers and monitors pulmonary tuberculosis (TB) patients for 2 years. Between May 2022 and November 2023, a total of 1,080 participants were enrolled in the study. As part of the cohort study enhancement strategy, the study established a foundation for optimizing personalized treatment strategies by analyzing treatment success rates, recurrence rates, and drug-related side effects. Additionally, the study aimed to investigate long-term sequelae such as post-TB lung disease and established a framework for leveraging advanced technologies, including chest X-rays, transcriptomics, and therapeutic drug monitoring. Isoniazid-resistant TB (Hr-TB) is the most common form of drug-resistant TB worldwide. In the Republic of Korea, Hr-TB prevalence tends to increase with age, underscoring the need for age-specific treatment strategies. Although a six-month regimen comprising rifampin, ethambutol, pyrazinamide, and fluoroquinolones is recommended for Hr-TB treatment, current treatment guidelines lack robust evidence due to insufficient randomized controlled trials. To improve Hr-TB treatment, the current project analyzed anti-TB treatment outcomes and side effects, proposing a personalized treatment algorithm tailored to patient conditions. Based on this approach, treatment strategies were stratified by risk of side effect due to pyrazinamide and TB severity, aiming to achieve optimized treatment and side effect management for individual patients. This approach can contribute to improving treatment success rates, avoiding extended treatment durations, and minimizing side effects. The 2024 revised Koirean TB guidelines incorporated the current study's findings, emphasizing “testing for fluoroquinolone resistance” and “shortened pyrazinamide usage duration”. The cohort study of tuberculosis participates as RePORT Korea within the RePORT Consortium, enabling Korea to play a pivotal role in TB research and strengthen international collaboration. This partnership facilitates cooperative efforts among domestic and international researchers to improve TB diagnosis, prevention, and treatment while enhancing research capacity. Data collected in Korea could serve as valuable resources for global TB research, enabling the country to demonstrate leadership in the global TB elimination effort. TB survivors often experience various sequelae and a decline in quality of life, necessitating long-term health management systems and evaluations for post-TB lung disease. Early detection of asymptomatic or subclinical TB is crucial, and the World Health Organization has introduced definitions and diagnostic frameworks to manage it effectively. This approach is expected to enhance TB management quality and significantly contribute to achieving global TB elimination goals.
