■ (Objectives and Necessity) This study was initiated in response to the growing need for a scientifically grounded framework to assess the health impacts of climate change and to develop effective policy measures. Building on the lessons learned from the First National Climate–Health Impact Assessment (’21), this research aimed to improve assessment domains, indicators, and analytical methodologies, and to provide a more precise evaluation of climate-related health risks. To achieve these goals, the study: ①reviewed international reporting frameworks to propose a structure for the Second Assessment that strengthens global comparability; ②established an application framework for advanced analytical methodologies to enhance the precision and validity of assessment results; ③examined the potential use of cohort data to reinforce scientific evidence; and ④reviewed strategies for updating and expanding assessment domains and indicators for the Second National Climate–Health Impact Assessment (’26). Additionally, ⑤expert advisory panels were convened and operated for each thematic area to ensure the appropriateness, validity, and applicability of assessment domains and methodologies, thereby improving the reliability and usability of the overall assessment framework. ■ (Major Findings) ① (Proposed Structure for the Second Assessment Report) Through a comprehensive review of major international reports (WHO, the United Kingdom, the Netherlands, the United States, and Germany), two report structures—“domain-oriented” and “expanded assessment”—were proposed. Policy-relevant implications were also refined, including evaluation of adaptation benefits, protection of vulnerable populations, and communication strategies, to support report development and policy integration. ② (Development of Advanced Analytical Methodologies) To enhance assessment precision, methodological frameworks incorporating state-of-the-art approaches were developed. For estimating excess mortality attributable to heatwaves and cold spells, advanced models such as the Distributed Lag Non-linear Model (DLNM) and Best Linear Unbiased Prediction (BLUP) were proposed. Future projection methodologies based on SSP–RCP scenarios were also established, providing a framework to estimate potential future excess mortality attributable solely to climate change under constant vulnerability assumptions. In the air-quality domain, methodologies for estimating the indirect mediated effects of fine particulate matter and ozone via temperature change were refined, along with new complementary indicators. ③ (Assessment of Cohort Data Utilization) The applicability of national cohort data—Korean Genome and Epidemiology Study (KoGES), National Health Insurance Service (NHIS) sample cohort, and Korea National Health and Nutrition Examination Survey (KNHANES) follow-up cohort—was reviewed. Cohort data were found to be valuable for overcoming limitations of ecological analyses, establishing individual-level exposure–response functions, and identifying vulnerable population groups. The study recommends conducting initial analyses at the municipal level and pursuing finer spatial linkage (eup-myeon-dong) through future Korea Disease Control and Prevention Agency–led research. ④ (Updating and Expanding Assessment Indicators) Strategies were developed to update and refine the existing indicators (31 indicators across heatwaves/cold spells, air quality, and infectious diseases). Candidate indicators for new assessment domains—including climate-related disasters (typhoons, heavy rainfall, wildfires), pollen, chronic diseases, and mental health—were reviewed, resulting in 46 proposed new indicators (total 77). Evidence supporting the relationship between climate factors and chronic diseases/mental health outcomes in Korea, including potential for calculating excess mortality and hospitalizations, was also identified. ⑤ (Expert Advisory Panels and Policy Recommendations) Expert advisory panels (one workshop, two forums, and one symposium) were convened for key themes to collect multidisciplinary expert opinions. Their input contributed to strengthening the rigor, feasibility, and policy relevance of the Second Assessment. ■ (Conclusion and Expected Impact) This study established a comprehensive foundation for expanding assessment indicators and advancing the framework for the Second National Climate–Health Impact Assessment. It offers methodological evidence to support a more scientifically robust assessment system and provides insights that can inform future national climate-health policies. The findings are expected to strengthen the policy relevance and practical utility of the Second Assessment, ultimately contributing to enhanced climate-health resilience and improved communication strategies for public health protection.
