Background - Heart failure is a chronic condition with high prevalence and mortality rates, leading to increased readmission rates and healthcare costs. Interventions utilizing digital healthcare technologies and wearable devices have the potential to enhance physical activity, improve medication adherence, and prevent disease exacerbation in heart failure patients. This study was designed to evaluate the clinical applicability of digital healthcare technologies. Objective - To assess the effectiveness of interventions using wearable devices and digital healthcare technologies in increasing physical activity and improving disease management in heart failure patients. Methods - A total of 600 patients hospitalized for acute heart failure were randomized into an intervention group (using wearable devices and a digital app) and a control group (standard care) and were followed for 12 weeks. The primary outcomes included quality of life (KCCQ, EQ-5D), physical activity (6-minute walk test), medication adherence (MMAS-8), biometric signals, and readmission rates. Results - Although there were no statistically significant differences in quality of life and medication adherence between the intervention and control groups, the intervention group showed greater improvements in both domains. Additionally, the intervention group demonstrated superior physical activity gains, as evidenced by the 6-minute walk test. Conclusion - Wearable devices and digital healthcare technologies can be valuable for personalized management and early detection of disease exacerbation in heart failure patients. They hold potential for reducing readmission rates and improving quality of life over the long term. Future studies should focus on long-term follow-ups to validate the effectiveness of digital healthcare technologies and refine strategies for adapting these technologies for elderly and digitally underserved populations. Significance - This study represents the first large-scale investigation in South Korea applying wearable devices and digital healthcare technologies to heart failure management. It highlights the potential for clinical adoption of digital healthcare technologies in cardiovascular disease management.
