As the importance of the Antimicrobial Stewardship Program (ASP) has been emphasized worldwide, the ASP pilot project has been implemented in Korea for medical institutions with more than 300 beds since November 2024. The purpose of this study is to identify the current status of domestic ASP at the time of implementation of the ASP pilot project and to derive indicators for ASP effect evaluation. A new questionnaire was developed based on a survey conducted in 2020 to understand the current status of domestic ASP at the time the ASP pilot project was implemented. The questionnaire was completed by reviewing the related literature, adding items that do not overlap with the existing questionnaire, and discussing items necessary to identify the current status of domestic ASP. The survey was conducted during January-February 2025, and was conducted on medical institutions with more than 300 beds nationwide. Of the 297 institutions, 153 (51.5%) responded, 71 (46.4%) participated in the ASP pilot project, and 82 (53.6%) did not participate. The main reasons for not participating in ASP were the shortage of dedicated ASP personnel (79.5%), shortage of doctors (60.2%), and shortage of pharmacists (57.8%). In the case of ASP-participating institutions, 56.1% of the budget was allocated to labor costs and 16.0% to medical staff incentives. The antibiotic guideline retention rate of ASP participating institutions was 84.5% and that of non-participating institutions was only 38%. The rate of implementation of the restricted antibiotic program was 100% in the participating institutions and 56.6% in the non-participating institutions. In the case of monitoring and recommending antibiotic prescriptions, ASP-participating organizations showed high performance rates in microbial-based antibiotic intervention (59.2%), specific antibiotic concentration monitoring (56.3%), and unnecessary combination therapy intervention (52.1%). In non-participating organizations, the performance rate of most items was less than 10%. To derive indicators for ASP effectiveness evaluation, candidate indicators were derived through a systematic literature review process, and three Delphi surveys were conducted on a panel of major ASP experts active in domestic professional associations. Through a systematic review of the literature, a total of 59 detailed indicators were extracted from a total of 7 major categories (antibiotic use-related indicators, clinical outcome indicators, microbiological indicators, economic indicators, process-related indicators, special departmental indicators, and other indicators), and the finally selected indicators from the 3rd Delphi survey were 1) DOT/patient-days, 2) DDD/patient-days, 3) wide-range antibiotic use reduction rate, 4) IV to oral antibiotic conversion rate, 5) unnecessary combination therapy reduction rate, 6) antibiotic prescription rate change according to WHO AWaRe classification, and 1) appropriate prescription rate for preoperative prophylactic antibiotic use before surgery, 2) recommendation for TDM and dose, and 3) guidelines for antibiotic use in the institution. In addition, in the case of 'special department-related indicators', 1) prophylactic antibiotic appropriate use rate before surgery, 2) appropriateness of the time to stop antibiotics after surgery, and 3) appropriateness of prophylactic antibiotic selection (when an infection occurred in a surgical patient) were selected. Finally, the core element performance was selected as an indicator for the evaluation of ASP effectiveness."
