Paxlovid (Nirmatrelvir/ritonavir) and Lagevrio (Molnupiravir), oral antivirals for COVID-19, have been shown to effectively prevent severe disease and mortality, serving as crucial pharmacological interventions during the pandemic. However, a comprehensive evaluation of their effectiveness and cost-effectiveness based on real-world data in South Korea has been limited. This study aimed to assess the prevention of severe and critical cases, analyze associated healthcare costs, and examine prescription inequities to provide evidence for future policy development and infectious disease preparedness. A unified database was created by integrating data from the Korea Disease Control and Prevention Agency (KDCA) and the Health Insurance Review and Assessment Service (HIRA). The study analyzed patients prescribed oral antivirals in 2022 and matched control groups of untreated patients using 1:1 propensity score matching (PSM) based on age, sex, comorbidities, vaccination status, and prior COVID-19 infection. Paxlovid and Lagevrio prevented 4,698 and 690 severe cases, 778 and 174 critical cases, and 1,513 and 319 in-hospital deaths, respectively. By age, Paxlovid showed significant effectiveness in individuals aged 60 years or older, while Lagevrio was more effective in those aged 70 years or older. Estimated reductions in direct medical costs due to disease prevention were \35.09 billion (severe cases), \42.74 billion (critical cases), and \8.22 billion (deaths) for Paxlovid, and \4.89 billion (severe cases), \9.19 billion (critical cases), and \1.76 billion (deaths) for Lagevrio. Prescription rates for oral antivirals were higher in clinics among medical institutions and in the metropolitan area, including Seoul and Gyeonggi, by region. Oral antivirals significantly reduced severe disease, mortality, and related healthcare costs, confirming their essential role in COVID-19 management. However, the high drug costs highlight the importance of targeted use in high-risk populations to optimize cost-effectiveness. Addressing regional and institutional disparities in prescription rates is crucial for equitable access to these treatments and for enhancing the country’s capacity to respond to future pandemics.
