Background & Objective: The prevalence and mortality rates of cardiovascular diseases (CVDs) among older adults are rising due to the increasing risk factors such as obesity linked to Western dietary habits and rapid population aging. This study aims to identify epidemiological characteristics of high-risk groups for CVD through a prospective cohort study of elderly participants in a preventive care cohort for severe diseases. By producing indicators for prevention and management, this research seeks to provide foundational data and implications for establishing national-level strategies for CVD prevention and management. Methods: This study is a community-based cohort follow-up study conducted over 10 years, targeting adults aged 65 years and older from 2012 to 2022. Initial baseline surveys (2012–2015) were followed by the first follow-up survey (2016–2019) and the second follow-up survey (2020–2022). Comprehensive data were collected on cardiovascular risk assessment, health behaviors, physical activity, and social networks. Data quality control and integrated database construction were performed for the cohort dataset accumulated over 12 years. Using this dataset, correlation analyses between social networks and the health characteristics of older adults were conducted, along with an analysis of risk factors for CVD occurrence. Results: A total of 3,517 participants took part in the baseline survey, with 2,520 participating in the first follow-up and 1,544 in the second follow-up survey. Participants who completed all follow-ups were younger, had higher educational attainment, and reported higher household incomes at the baseline compared to those who only completed the baseline survey. Those with higher participation frequency exhibited greater thigh circumference, stronger grip strength, and shorter times on the TUG and CRT tests. Social network characteristics revealed smaller networks with increasing age and significant differences in network size and composition by gender. Among the baseline participants, risk factors significantly influencing CVD events (heart failure, stroke, atrial fibrillation, myocardial infarction) included blood pressure, body mass index, and low grip strength. However, due to the limited number of valid cases, further in-depth analysis is needed. Expected effect: This study systematically collected multi-faceted data through follow-up surveys on physical and mental health, socioeconomic factors, and environmental influences of elderly participants. By identifying changes in health and associated factors among high-risk groups for CVD, this study aims to produce indicators for effective prevention and management. Ultimately, the findings are expected to contribute to the development of effective national prevention and management strategies for CVD among older adults.
