Development of drug candidates against carbapenem-resistant Acinetobacter baumannii by drug repurposing

This study was conducted to identify and evaluate effective compounds for treating carbapenem-resistant Acinetobacter baumannii (CRAB) infections. A chemical library comprising 6,158 compounds was established, and a screening method was developed to assess antimicrobial activity and carbapenem adjuvant activity using a susceptible strain (ATCC 19606) and a resistant strain (CRAB57). Primary screening identified 71 compounds that inhibited the growth of A. baumannii, and after considering toxicity and clinical applicability, five antimicrobial compounds and one carbapenem adjuvant compound (KS-2) were selected. Notably, KS-2 was identified as azithromycin, an existing antibiotic that exhibited a synergistic effect with imipenem. A testing method was established to assess the susceptibility of A. baumannii, and susceptibility testing on 89 clinical isolates collected from a general hospital in Korea revealed that the resistance rate decreased to 53.9% in RPMI medium. Checkerboard assays confirmed the synergistic effect of azithromycin and imipenem in three out of five clinical isolates. These findings suggest that azithromycin, when combined with carbapenems, may enhance the treatment potential for CRAB infections. Furthermore, transcriptomic analysis of CRAB treated with azithromycin and imipenem indicated that further investigation at the protein level is required to elucidate the mechanism of action. To facilitate in vivo studies, a CRAB strain expressing a bioluminescent protein gene was constructed, providing a foundation for evaluating infection and treatment efficacy in animal models. The chemical library and screening techniques developed in this study can be applied not only to CRAB infection treatment but also to the development of antibiotics for other pathogens. In particular, the confirmed carbapenem-adjuvant effect of azithromycin suggests that with further validation of its efficacy and safety, its clinical applicability could be significantly enhanced.
