Background and Purpose: Amyloid-beta begins to accumulate in the brain 10-15 years before cognitive decline, indicating that dementia progresses slowly even in cognitively normal individuals.. However, there has been a lack of research on monitoring and predicting dementia or its relationship with other chronic diseases. This study aims to investigate the association between chronic diseases, such as cardiometabolic diseases(diabetes, hypertension, dyslipidemia), and dementia, and to identify intervention target indicators. Methods: Using resources from the Korea Urban Rural Elderly study (KURE), we investigated blood dementia-related indicators (Ab40, Ab42, GFAP, NFL) and linked them with health insurance claim data. We performed logistic regression and Cox proportional hazards model analyses. Additionally, we conducted preclinical animal model and mechanism studies, including high-fat diet and hypertension (AngII infusion) models. Results: Among the blood dementia-related indicators (Ab40, Ab42, GFAP, NFL), significant results were derived from the blood NFL (neurofilament light chain) indicator, which reflects neurological damage. In a cross-sectional study, blood NFL was divided into tertiles, and the highest tertile showed a 29% increased risk of diabetes (Odds ratio 1.29) compared to the lowest tertile. Analysis of the association between NFL and dementia incidence and mortality revealed that the highest tertile had a 2.12-fold increased risk of dementia or death within 10 years compared to the lowest tertile (HR 2.12). However, there was no significant association with the risk of severe CVD. In preclinical models, high-fat diet increased BACE1 and decreased ZO-1 protein in brain tissue, indicating a loss of endothelial characteristics and a shift towards mesenchymal traits, as evidenced by increased SMA (Smooth Muscle Actin) and N-cadherin. Linking clinical epidemiological analysis with preclinical models, we confirmed that blood NFL increased with aging, although there was no difference in NFL levels induced by high-fat diet or hypertension (AngII). Conclusion and Significance: High blood NFL levels were found to be associated with current diabetes status and an increased risk of dementia and death within 10 years. Additionally, high-fat diet induced endothelial-mesenchymal transition in brain vasculature, although further research is needed. These results suggest that measuring NFL in the early stages of hypertension and other precursor diseases may be useful as a predictor of dementia incidence and mortality.
