The COVID-19 pandemic that emerged in 2020 represented a race between rapid vaccine and antiviral development and the virus’s continuous evolution. With increased population immunity through vaccination and natural infection, SARS-CoV-2 has become an endemic virus. In modern medicine, the use of agents such as Rituximab, CAR-T, and bispecific T-cell antigen engagers has increased the number of patients with B-cell dysfunction, who may experience persistent COVID-19—a condition characterized by prolonged viral presence and recurrent inflammation. These cases have raised concerns about potential variant emergence and became a major healthcare issue in Korea since 2022. Although passive immunization with convalescent plasma appeared effective in case reports, strong clinical evidence was lacking. This project aimed to provide scientific proof of its efficacy through a randomized controlled trial (RCT). Given the limitations of convalescent plasma, attention shifted to IVIG, which—due to widespread vaccination and hybrid immunity—was found to contain high neutralizing antibody titers against SARS-CoV-2 and offered stable supply. Preliminary studies confirmed its neutralizing activity, leading to IND approval within four months. Early enrolled patients showed clinical improvement consistent with prior reports. However, as persistent COVID-19 cases markedly declined after 2023, it became impractical to complete the planned RCT. Following approval from the commissioning department, the study focus was revised to evaluate antibody and T-cell immune responses in B-cell–deficient patients during the endemic phase. Patients who developed B-cell dysfunction before the endemic period showed impaired antibody formation and sometimes persistent infection, but maintained cellular immunity after recovery. Those who began B-cell–depleting therapy during the endemic phase exhibited lower antibody titers but preserved T-cell responses. Currently, persistent COVID-19 is no longer observed in Korea. This experience highlights the need to establish a readily deployable passive immunization system for future emerging viral pandemics, to be maintained until viral elimination or endemic transition.
