This study systematically evaluated the effects of short-term and long-term exposure to fine dust (PM10) and ultra-fine dust (PM2.5) on the incidence and exacerbation of major diseases using nationwide health insurance claim data and health examination data. High-resolution air pollution exposure data at the city/county/district level were linked, and short-term exposure was analyzed using a time-stratified case-crossover design, while long-term exposure was analyzed using cohort-based survival analysis. In the early stages of the study, a multidisciplinary advisory panel comprising 98 experts from 16 specialized fields, including environmental epidemiology, clinical medicine, and data science, participated. Through systematic literature reviews, knowledge-sharing seminars (total of 23 sessions), and structured discussions, the panel gained a thorough understanding of the data structure and characteristics. 208 research questions and 80 priority diseases were selected based on exposure duration (short-term/long-term) and health outcome (onset/worsening) types. Actual analysis was conducted based on this predefined topic selection framework, and the results confirmed that PM exposure significantly influences the onset and worsening of various disease groups, including cardiovascular, respiratory, mental health, ophthalmic, skin, and renal diseases. Additionally, this study presented the potential for utilizing the databases used in the analysis in clinical and epidemiological studies, as well as options for public disclosure to researchers. Currently, access is limited to specific research institutions in accordance with relevant procedures, but it is anticipated that gradual expansion of public disclosure will be possible through coordination among relevant government agencies in the future. This study represents one of the first attempts in Korea to comprehensively analyze the health effects of fine dust by combining large-scale population-based data with multidisciplinary expertise. It is expected to serve as an important evidence-based resource for the development of environmental health policies and disease-specific response strategies in the future.
