The COVID-19 vaccine is known to be an effective way to prevent and reduce the severity and death of COVID-19. Since the development of the COVID-19 vaccine was completed in a relatively short period of about a year, and it received rapid approval and began to be used, the operation of an active monitoring system for unexpected side effects that may occur after vaccination was strongly recommended. On April 7, 2021, the European Medicines Agency reported that a very rare side effect of thrombosis with thrombocytopenia may occur in about 10 cases per million shots as a very rare side effect after the AstraZeneca vaccine, and it was accepted that the same rare side effect could be occured in Janssen vaccine. In Korea, the COVID-19 vaccination was started on Febrary 26, 2021. In the early days, AstraZeneca vaccine was the main vaccine, so a vaccine adverse reaction monitoring system was operated for the possibility of side effects of this rare side effect. The centralized test request and monitoring system was created using the anti-PF4-antibody ELISA test method, which is an autoantibody against platelets, known as the mechanism of this very rare side effect (thrombocytopenia-thrombosis syndrome; TTS). From November 1, 2022, to December 31, 2023, the anti-PF4-antibody ELISA test was conducted. There was one case recorded on September 11, 2023 (sample collected on September 5, 2023). The patient had a history of receiving the Pfizer vaccine on January 6, 2022. She visited the clinic due to dyspnea in July 2023 and, during outpatient care on July 31, experienced fever and decreased oxygen saturation, leading to emergency room treatment followed by blood tests and radiological examinations. Although she showed a decrease in platelets of less than 150K and a d-dimer value increased more than four times the normal range, the timing of the symptoms did not align with TTS and there was no evidence of thrombosis in the radiological examinations. The result of the anti-PF4-antibody ELISA test was also negative. The number of requests for the anti-PF4-antibody ELISA test decreased from a total of 43 cases in 2022 to 1 case in 2023. Considering that rare adverse reaction, TTS, is acknowledged in the country after using adenovirus COVID-19 vaccines such as AstraZeneca or Janssen, and there are no plans for the use of these vaccines domestically, given that confirmed cases of TTS after AstraZeneca vaccination in the country total 4 cases (0.2 cases per million AstraZeneca vaccinations), it is deemed that the need for an operational surveillance system, including expert consultation, would be minimal. Despite showing low correlation, the necessity of administering improved vaccines (mRNA vaccines) that can counter viruses with various variants continues to be emphasized. Therefore, it appears necessary to continue operating the established anti-PF4-antibody ELISA test in the central laboratory. Following the anti-PF4-antibody ELISA test, platelet function test setup utilizing normal platelets has been planned to establish a testing system that enhances diagnostic specificity.
