초록

1. Final Objective - This study aims to establish a multicenter cohort by collecting information from institutions providing assisted reproductive technology (ART), childbirth, neonatal, and pediatric care for infertile patients. The primary goal is to construct a long-term cohort encompassing basic information about couples, details of infertility procedures, infertility causes, ovarian stimulation methods, oocyte retrieval and embryo generation, embryo transfer details, pregnancy outcomes, neonatal health status, and maternal health tracking. To achieve this, a large-scale retrospective data analysis will be conducted to gather preliminary information for designing a prospective cohort. This information will then be applied to a pilot study for a prospective cohort. 2. Domestic and International Technological Status - While the importance of maternal and child cohort studies has been recognized globally for over two decades, leading to large-scale studies supported by national governments, domestic cohort research related to infertility and obstetrics remains limited. There is a particular lack of integration with infertility treatments in existing studies. 3. Final Research Content and Methodology - Based on literature review findings, a process for building retrospective and prospective datasets has been designed, and the collection process has been optimized. 4. Final Research Results (1) Retrospective Cohort Study -EMR Data: Bundang CHA Hospital: 406 infertility treatment cases, 371 controls. Gangnam CHA Hospital: 249 infertility treatment cases, 293 controls. -CDM Data: Bundang CHA Hospital: ~10,000 individuals. Kyungpook National University Hospital: 89 individuals. Seoul National University Bundang Hospital: 2,170 individuals. -NHIS Data: Nationwide data for all infertility treatment cases post-2017 (230,147 individuals). (2) Prospective Cohort Study - Design and Features: A multidisciplinary protocol was developed, incorporating clinical, emotional, social, and environmental factors. A pilot study (n = 200) was conducted using the iCReaT system and ResearchFlow self-reported survey services for efficient data management and follow-up. - The main protocol was established by incorporating improvements identified from the pilot study (the protocol is attached as a supplementary document). 5. Discussion and Conclusion - Analysis of EMR and HIRA data revealed that the ART group had higher rates of uterine and ovarian diseases compared to the control group. The average age of the ART group was 35.6–40.14 years, which was higher than that of the control group. Among women who successfully conceived, the average age was 39.61 years. IVF (in vitro fertilization) demonstrated the highest pregnancy success rate among ART methods. EMR and HIRA data showed similar trends, while CDM data presented some limitations for conducting infertility treatment studies. - Standardization: Develop standardized EMR templates to enhance the efficiency of data collection and follow-up observation through data integration across participating institutions. - Workload Optimization: Delegate data entry and specimen management to specialized agencies to alleviate the workload of clinical research coordinators (CRCs) and improve work efficiency. - Long-Term Follow-Up: Extend the study duration to track maternal health outcomes and the growth and development of children born following ART over the long term. - Participation Incentives: Increase financial incentives for survey and follow-up test participation to improve participation rates (e.g., small-scale monetary compensation at each stage). -Central IRB: Establish a centralized IRB system to streamline the approval process across research institutions.
