Influenza is responsible for a substantial burden of disease and mortality worldwide. Given its wide clinical spectrum from mild cases to severe complications and death, influenza-related mortality may often be underestimated, especially when it leads to the worsening of underlying diseases or is not recorded as the direct cause of death. This study aimed to estimate the seasonal excess mortality and mortality risk associated with influenza in the Republic of Korea, with a particular focus on the impact of SARS-CoV-2 co-infection. A nationwide retrospective cohort study was conducted using the National Health Insurance Service claims database. Individuals with a diagnosis of influenza (ICD-10 codes J09-J11) during the 2016/2017 to 2022/2023 seasons were included and matched 1:2 with individuals without influenza based on sex and age. Influenza-related mortality was defined as death within 30 days after the index date. Excess mortality was calculated as the difference in crude mortality rates between the influenza and control groups, and odds ratios (ORs) with 95% confidence intervals (CIs) were estimated using conditional logistic regression. Mixed-effects model was used to estimate pooled odds ratios across seasons. The pooled 30-day influenza-related excess mortality was 48.3 per 100,000 individuals across the study period, and 46.0 per 100,000 after excluding COVID-19 pandemic seasons. The influenza-associated mortality risk was significantly elevated (pooled OR=3.35, 95% CI=2.84-3.96), with the highest excess mortality observed in individuals aged ≥65 years (825.7 per 100,000) and those with chronic comorbidities (e.g., cardiovascular disease, chronic kidney disease, diabetes, atrial fibrillation, and transient ischemic attacks). Furthermore, we examined clinical outcomes among patients with influenza and COVID-19 co-infection. During the 2021/2022 season, when both viruses were circulating, co-infected individuals showed a higher, though not statistically significant, risk of 30-day mortality compared to those with influenza alone (OR=1.90, 95% CI=1.03, 3.51). No significant associations were observed for 60-day or 90-day mortality. In contrast, during the 2022/2023 season, a period with reduce d COVID-19 incidence and enhanced public health measures, the difference in mortality risks between co-infected and influenza-only groups was minimal. These findings highlight the potential role of co-infection in acute clinical deterioration and underscore the importance of ongoing surveillance and targeted interventions, particularly among high-risk populations. Further studies are warranted to evaluate the effect of vaccination history, antiviral treatments, and the long-term outcomes of co-infected individuals.
