Establishment of a pilot operation system for incident-based and syndrome-based surveillance in preparation for cluster outbreaks of unknown etiology

Background and Necessity -The risk of emerging and unknown infectious diseases has significantly increased due to the COVID-19 pandemic, climate change, and global mobility. -Korea’s current surveillance system is based primarily on legally notifiable diseases (Indicator-based Surveillance, IBS), which limits early response capability for unknown diseases or atypical cluster outbreaks. -To address these limitations, this study aims to introduce Event-based Surveillance (EBS) and Syndromic Surveillance (SS) systems to enable real-time anomaly detection and early warning of potential outbreaks. Objectives -Analyze international surveillance systems (CDC, UKHSA, WHO, etc.) and derive applicable strategies for domestic implementation. -Establish a data collection and preprocessing framework integrating domestic health data sources (HIRA, NEDIS) and online data (social media, news, academic articles, international reports). -Develop time-series and machine-learning–based anomaly detection algorithms and an AI-driven automated surveillance system. -Validate the proposed pilot operation system through domestic case applications, including Monkeypox and Mycoplasma pneumoniae outbreaks. Methods -Data Collection: 1,828 disease categories from HIRA open data (2010–2024) and approximately 220,000 records from SNS, online communities, academic papers, and international organizations were automatically collected. -Time-Series Analysis: Change-point detection (PELT, Bayesian CPD), scenario forecasting (ARIMAX, structural time-series), and disease-pattern clustering (six types). -AI Surveillance System: Event classification into seven categories (infection, spread, symptom, prevention, control, recovery, death) using GPT-4.1-nano, applying confidence-weighted analysis. Peak points on time-series curves were identified as anomalies triggering early warnings. Key Results -Disease Pattern Analysis: - Circulatory and musculoskeletal diseases showed a balanced trend of increase and decrease. - Respiratory diseases showed a higher proportion of decreasing or volatile patterns, indicating possible effects of public health interventions. -AI Surveillance Pilot: - Monkeypox: Detected an increase in “symptom + recovery” event mentions about 2–3 months before the actual outbreak. - Mycoplasma pneumoniae: Identified anomaly signals 1–2 months prior to the observed surge. -International Trend Analysis: Using TF-IDF, KeyBERT, LDA, and BERTopic, major research trends were identified in infectious diseases, cancer, cardiovascular diseases, and AI-based medical technologies. Conclusion and Implications -This study established the nation’s first integrated pilot system combining Event-based and Syndromic Surveillance, -demonstrating the feasibility of an AI-driven early warning system for unknown disease outbreaks. -Future directions include expanding real-time data linkage (NEDIS, HIRA, SNS), advancing LLM-based detection models, -and strengthening legal and institutional foundations to evolve into a national, real-time infectious disease early-warning system.
