A study on the effect of antibiotic use on the intestinal microbiome

○ This study investigated the impact of antibiotic use on the gut microbiome of patients with carbapenem-resistant Enterobacterales (CP-CRE) using longitudinal fecal samples collected along the treatment course. Gut microbiome profiling was performed using full-length 16S rRNA sequencing and whole-metagenome shotgun sequencing in 46 CP-CRE patients, with 40 healthy individuals included as controls. ○ Compared with healthy controls, CP-CRE patients exhibited reduced microbial diversity and distinct community structures, particularly during antibiotic treatment. Antibiotic use was associated with an increased relative abundance of opportunistic pathogens such as Enterococcus and Klebsiella, whereas commensal taxa including Faecalibacterium and Blautia were depleted. Partial recovery of microbial diversity and community structure was observed after antibiotic discontinuation, while antibiotic switching resulted in alternative restructuring patterns, indicating inter-individual variability in microbiome resilience. ○ Functional profiling revealed that antibiotic exposure broadly suppressed core microbial pathways related to nucleotide biosynthesis, energy metabolism, and cell-wall synthesis, with recovery following antibiotic discontinuation. Antimicrobial resistance (AMR) analysis showed higher contributions of resistance mechanisms during antibiotic use, which decreased after discontinuation. Short-chain fatty acid analysis further demonstrated significantly reduced acetate and propionate levels in antibiotic-exposed CP-CRE patients compared with healthy controls. ○ To translate microbiome findings into therapeutic resources, a culturomics-based workflow was established to isolate and screen candidate strains. Lactiplantibacillus plantarum strains exhibited strong anti-CRE activity, including antibacterial, anti-biofilm, and anti-adhesion effects. Whole-genome sequencing of selected candidates identified bacteriocin-related gene clusters without detectable virulence or antibiotic resistance genes, supporting their potential as safe live biotherapeutic candidates.
