1. R&D objectives: This project aim to maintain and conduct follow-up research on the already established "Cardiovascular and Metabolic Disease Etiology Research Center (CMERC) Cohort," and plan a long-term follow-up investigation strategy. The goal is to conduct follow-up surveys targeting 1,200 participants (including 1,000 face-to-face surveys and 200 non-face-to-face surveys) from the CMERC cohort based investigation (2013-2018) and secure clinical, epidemiological, imaging data, and human biological resources from this cohort that has been followed for more than 10 years. 2. R&D Contents ㅇ Collection of Epidemiological, Clinical Information, and biosamples: The project obtained consent from CMERC baseline survey (2013-2018) participants for follow-up investigations and conducted follow-up surveys on 1,000 individuals, including both the general population and high-risk groups. It secured clinical, epidemiological, imaging data, and biological samples. Standardized protocols were developed, and survey staff received training. For face-to-face surveys, participants provided written consent in advance, visited medical institutions for health screenings, and contributed additional clinical and epidemiological data. Biological samples (3 ml of serum, 3 ml of plasma, and 1 ml of buffy coat) were collected and deposited in the National Biobank of Korea. Imaging data (QCT, IMT, OCT, etc.) were extracted from hospital information systems, anonymized, and uploaded to a research data lake. Progress on follow-up examinations has been reported periodically, and data collected for secondary analysis were uploaded to the Cohort Epidemiology System of the Korea NIH. ㅇ Long-Term Follow-Up Strategy: Research outcomes derived from linking CMERC baseline data with secondary hospital data were analyzed to enhance the utility of follow-up data. Characteristics of participants and non-participants in follow-up surveys were compared. A non-face-to-face survey platform was developed and pilot-tested to gather useful research data from non-participants. Domestic and international cohort studies employing face-to-face and non-face-to-face methods were analyzed for comparative purposes, and strategies for maximizing the utilization of CMERC resources were proposed. Additionally, analyses linking health insurance claims data were initiated to identify risk and preventive factors influencing cardiovascular and metabolic diseases. 3. R&D Achievements: Over a total of three years, including two years of prior research and the current study, follow-up investigations were completed for 3,692 participants. Newly obtained consent enables third-party utilization of research resources, and additional clinical, epidemiological, imaging data, and biospecimens were collected and deposited at the Korea NIH. The remote follow-up survey was conducted by sending a survey request via text message to 538 individuals, of which 418 completed the survey. Strategies for mid- to long-term follow-up investigations and the continuous acquisition of research resources were proposed to further develop the cohort as a foundation for providing scientific evidence on the prediction and prevention of cardiovascular and cerebrovascular diseases in Koreans. 4. Utilization and Expected Effects: This research project has developed a platform for long-term follow-up surveys and resource acquisition of previously established cohort participants. It is expected that these efforts will provide foundational data for the development of health policies aimed at preventing cardiovascular and metabolic diseases. The systematically collected high-quality data and biological resources will be utilized as national research resources, distributed to researchers, thereby promoting research in related fields and contributing to the enhancement of public health.
