초록

ㅇ연구의 배경 및 목적 - 국내 삼일열말라리아는 1993년 재출현 이후 30여년째 고위험지역을 중심으로 지속적으로 발생하고 있으며, 코로나19 대유행 이후 해외유입 말라리아 환자도 증가하고 있음. - 말라리아 진료가이드 개발을 통해 말라리아 환자를 신속하게 진단하고 치료하여 말라리아 전파를 조기에 차단하고 국민 건강을 보호함. - 연구의 목적: ▲국내 삼일열 말라리아의 예방 및 조기 치료를 통해 국내 말라리아 퇴치에 기여하고 ▲다양한 말라리아 종의 특성을 반영한 해외유입 말라리아의 적절한 치료법을 제시하며 ▲국내에서 사용 가능한 항말라리아제와 최신 국제 지침을 기반으로 실제 임상 현장에서 활용 가능한 표준화된 진료 체계를 확립함. ㅇ연구 방법 - 국외 말라리아 진료지침 검토하여 핵심 임상 질문 도출 - 핵심 문헌고찰 (신속 고찰 방법을 따름) - 이에 따라 진료 기준 치료 지침에 대한 권고안 도출, 권고안 근거문 작성 ㅇ연구 결과 - 국내 말라리아 진료 가이드를 치신의 근거와 국제 권고를 반영하여 개정함. - 피로나리딘-알테수네이트의 임상적 활용 가능성, 타페노퀸 사용, 국내 말라리아 예방의 근거 마련 등은 이번 개정 과정에서 중요한 고려 대상이었음. - 다수 전문가들이 참여한 문헌 고찰과 합의 과정을 거쳐, 임상에서 활용가능한 진료 가이드를 도출하여, 국내 말라리아 관리의 질적 향상에 중요한 기여할 것으로 생각됨 - 소아 및 임산부 등 특수 환자군의 치료 지침(약의 용량 및 용법)을 도출함　 - 해외유입 말라리아 조기 치료 및 사망 최소화를 위한 신속 대응 가이드 마련 - 국내 및 해외 말라리아 예방 약물 복용 근거 수준 결정 및 권고안 마련 ㅇ 기대 효과 및 활용 전략 - WHO 최신 권고와 근거 기반 의학을 반영하여 국내 임상진료에서의 표준성을 강화함. - 국내 삼일열 말라리아의 특성과 해외 유입 말라리아 대비를 동시에 포함하여, 지역적 특수성과 국제적 흐름을 균형있게 반영. - 예방, 진료, 치료 전 과정을 포괄하는 알고리즘을 제시하여 임상 현상의 활용성을 높이고, 국내 말라리아 관리의 질적 향상에 중요한 기여를 할 것으로 생각됨. - 활용 전략: 말라리아 관리지침에 반영, 의료진용 홍보 자료 제작, 학술지 게재 예정.

ㅇ Background and Objectives - Development of national malaria clinical guidelines to enable rapid diagnosis and treatment, prevent transmission, and protect public health Objectives: (1) Contribution to malaria elimination in Korea through prevention and early treatment of P. vivax malaria; (2) Presentation of appropriate treatments for imported malaria reflecting species characteristics; (3) Establishment of standardized clinical practice based on available antimalarials and international recommendations ㅇ Methods - Review of international guidelines to derive key clinical questions, Rapid literature review - Development of recommendations and supporting evidence summaries for clinical practice, Revision of the national guideline reflecting current evidence and international standards ㅇ Outcomes and Utilization Strategy - Consideration of pyronaridine–artesunate use, tafenoquine introduction, and malaria prophylaxis evidence - Production of practical guidelines through literature review and expert consensus - Establishment of treatment recommendations for children, pregnant women, and other special populations - Proposal of a rapid response framework for the early treatment of imported malaria - Determination of evidence levels and recommendations for malaria prophylaxis domestically and abroad - Strengthening of standardization in clinical practice by adopting WHO recommendations and evidence-based medicine - Balancing of domestic P. vivax malaria management with preparedness for imported malaria - Presentation of comprehensive algorithms for prevention, diagnosis, and treatment - Utilization strategy: Integration into national malaria management guidelines, development of educational materials, and publication in an academic journal.
