BACKGROUND · Improving treatment adherence and treatment success rates through effective TB patient management are critical components of TB control strategy. To this end, a public-private partnership TB management (PPM) program was first introduced in Korea in 2006 and scaled-up as nationwide program in 2019, effectively closing the gaps in TB patient management in the private sector by increasing treatment success rate from 77.9% to 93.5%. However, service access issues, job security of key medical personnel, governance structure, and inter-agency cooperation are important areas that require restructring and strengthening to cope with the changing future dynamic of the TB epidemiology in Korea. · (Research Needs) There is a need to develop additional indicators to address gaps in existing TB service quality and performance evaluation. These include: (1) development of methods to systematically assess workloads of dedicated healthcare personnel (TB nurses) and the quality of the programmatic infrastructure of PPM institutions; and (2) development of methods to assess of TB service delivery and operational efficiency. · (Purpose of the study) The main objective of our study is (1) to assess the programmatic infrastructure of the TB services and workloads of key clinical personnel at PPM institutions, and develop strategies to routinely monitor the quality and workloads; (2) to assess the institution-level operational efficiencies based on the routine data sources collected by PPM institutions and develop an evaluation framework for annual operational performance monitoring. METHODS A range of routine data, including TB notification data from 2019 to 2023, were consolidated and assessed at the institutional level to measure institutional efficiency and workloads of TB nurses. Data that were not routinely collected, including measures workload, were collected through a standardized survey instrument developed by our team. The survey instrument consisted of four domain areas: capacity of TB management organizations, medical staffing, means of TB management, and TB management workload and work ratio. TB nurses' workload was measured and compared based on three different calculation methods. The first method was based on a top-down approach, which divided total the number of patients by total number of nurse rosters allocated to each institution. The second metric included the concept of person-time of actual nurse employment, where subtracted the total person-months of nurse rostser vacancy in calculating workload in terms of the number of TB patients managed per TB nurse. The third approach further included the concept of total person-months of all TB patients in care at each PPM institution in addition to the second approach. In evaluating the institutional performance and operational efficiency, we developed efficiency scores for each PPM institution estimated based on the Data Envelopment Analysis (DEA), where we considered three different types of output variables (treatment success, total number of TB patients, total number of patients considered for vulnerability score assessment) and six input variables (including number of physicisans, nurses, annual budget) with each PPM program participating year and institution as individual management unit (total of 797 units). RESULTS · A total of 113 PPM institutions participated (64.9% participation rate) in the survey. When examining the tasks of TB nurses, we found that TB patient management, administrative tasks, management of latent TB patients and screening, and non-TB tasks accounted for the high proportion of regular duties of PPM nurses. TB nurses committed approximately 5.7 hours per patients for direct patient interaction for drug susceptible (DS) TB patients. This was 3.12 time higher for Multi-drug Resistant (MDR) TB patients, In addition, managing non-compliant TB patients took it required 1.7 times as much time as DS-TB patients whereas managing latent TB patients required 1/4th of time of the DS-TB patients. When comparing the three indicators for measuring nurse workload, there was a high correlation across different indicators. Relative to measuring workloads based on TB nurse placement criteria and number of notified TB patients at each PPM institution, applying real-world workload levels (i.e. measurements based on actual employment status and individual patients’ treatment duration), realved original workload indicated likely under-estimated the actual workloads. · The operational efficiency estimates showed that highly efficient institutions (i.e., those with an efficiency index of 0.8 or higher) are effectively utilizing key resources such as allocated budgets and medical staff to improve treatment outcomes for TB patients. At institutions with low-efficiency scores, we found there were trends of potential excessive nurse staffing or poor treatment outcomes. Furthermore, year-by-year observations of DEA-based efficiency score suggest that institutions with consistently low efficiency have not responded adequately to changes in nurse staffing levels and changes in patient volumes compared to those with higher efficiency scores. CONCLUSIONS We found that the workload metrics that reflect actual TB nurse employment status and TB patients treatment duration most accurately reflect the real-world workloads experienced by TB nurses at PPM institutions. Likewise, routine monitoring of TB nurse employment status and assessment of workloads based on month-to-month TB patient volumn is essential to better assess workloads faced by TB clinical personnel at the PPM institutions. Furthermore, our DEA based approach in evaluating operational efficiencies of TB programs at PPM institutions adequately distinguished institutions that may require close monitoring of operational performce by identifying differential needs at each institutions – from budget and resource input supports to strengthening staff management practices and TB service process improvements. These evaluation framework may be extended to assess and monitor the quality of TB services and operations of TB control programs on a regional basis, factoring demands of various forms of TB services including LTBI and TB screening as well as treatment and management.
