ㅇStudy Background Malaria remains a persistent public health challenge in Korea, with more than 600 cases reported annually and geographic expansion toward the Seoul metropolitan area. These trends hinder progress toward meeting WHO malaria elimination certification requirements. Current response systems show limitations in proactively addressing diverse transmission pathways—including asymptomatic infections, clustered outbreaks, and imported cases. Moreover, the designation of risk areas has relied largely on qualitative judgment, underscoring the need for a quantitative risk assessment tool and stratified response strategies to guide evidence-based decision-making. ㅇStudy Objectives The study aims to conduct a comprehensive analysis of malaria incidence and its associated risk factors at the local administrative level and to develop a quantitative risk assessment tool and a stratified malaria risk map. Building on this analytical foundation, the study seeks to establish risk-tier specific response strategies and an operational standard manual that enhance Korea’s malaria surveillance and control framework in alignment with WHO elimination standards. Specific goals include: (1) identifying and analyzing regional malaria risk factors and developing a quantitative risk assessment tool; (2) producing a spatially stratified risk map and standardized regional response strategies; and (3) identifying region-specific high-risk population groups and developing tailored strategic guidelines for each group. ㅇStudy Methods The study will analyze malaria case distribution and characteristics of affected populations across regions, identify local risk factors, and develop a risk-map modeling framework. Consultations with local health officials will be conducted to capture unmeasured contextual risk factors and available resources that influence regional vulnerability. Using this information, the study will establish a set of risk-tier specific response strategies and develop a standardized, detailed operational manual that reflects regional conditions and the needs of different high-risk population groups. ㅇExpected Outcomes The study will produce a comprehensive final research report containing: (1) a quantitative malaria risk assessment tool; (2) a stratified malaria risk map for local administrative units; (3) standardized response strategies and an actionable implementation manual; and (4) a summary report based on key informant interviews. These outputs are expected to support a more systematic, data-driven approach to malaria control and contribute to Korea’s progress toward elimination and strengthened national surveillance capacity.
