이 중 PKD1 truncating mutation 이 44%로 가장 많았고, PKD1 non-truncating muation이 22%, PKD2 mutation이 15%, PKD1- inframe indel이 5%, Minor genotype이 9%, No variant 나 VUS는 5%였다.

The goal of this study was to establish a cohort of patients with hereditary cystic kidney disease, build a diagnostic algorithm to identify the causative genes of patients, and secure human resources and clinical information. In addition, the prognosis of patients according to their genotype was analyzed to enable diagnosis and personalized treatment of this disease. During the three-year study period, a total of 804 patients (739 adults and 65 children) were enrolled from 13 centers in 11 institutions nationwide, and 781 patients are being followed up, excluding 23 dropouts. Of the 804 patients, DNA was available for 770, clinical information was available for 798, imaging was available for 782, and serum and urine samples were available for 274. The cohort was stratified into typical ADPKD, atypical ADPKD, and pediatric/other cystic kidney disease To genetically characterize the patients, a gene panel of 89 genes known to be associated with ciliopathy was constructed and used as a primary genetic screen. Undiagnosed patients underwent secondary genetic testing. In adults, the primary genetic analysis resulted in a mutation detection rate of 68.6%. When secondary genetic analysis was performed using LR-PCR PKD1 targeted exome sequencing, MLPA, Minigene assay, WES, and family segregation study, the mutation detection rate was 80.4%. In 573 adult typical ADPKD patients, genetic analysis identified pathogenic variants in 95.3% of cases. Of these, PKD1 truncating mutation was the most common (44%), followed by PKD1 non-truncating mutation (22%), PKD2 mutation (15%), PKD1-inframe indel (5%), minor genotypes (9%), and no variant or VUS (5%). Genetic analysis of 53 adult atypical ADPKD patients revealed a pathogenic variant in 65.9% of cases. Of these, minor genotypes accounted for 52%, and NV or VUS accounted for 34%. The correlation between genotype and prognosis of renal disease showed that PKD1-PT genotype showed significantly faster renal function decline than other genotypes, whereas minor genotypes showed stable renal function with almost no renal function decline. Analysis of extra-renal complications showed that in Korea, patients with PKD1 and PKD2 mutations had a higher incidence and severity of hepatic cysts. Mutations were identified in 85 (62.0%) of 137 pediatric patients (phase 1 and 2 pooled analysis), of which 55 (64.7%) were PKD1 mutations. In pediatric patients, genetic testing after clinical diagnosis resulted in a change in diagnosis in 39 (34.5%) patients, and of the 24 patients classified as nonspecific cystic kidney disease, 6 had mutations in PKD1 and 3 each in HNF1B, PAX2, and GANAB. Of these, the genotype of the PAX2 variant was associated with the expression of ophthalmologic symptoms, severity of symptoms, and long-term renal prognosis. This study demonstrated that a panel of cystogenesis-related genes is useful as a screening test, and that secondary genetic testing can increase the diagnosis rate to 80%. For the 20% of undiagnosed patients, long read sequencing for typical ADPKD patients and whole genome sequencing for atypical ADPKD or pediatric ADPKD patients may improve the diagnosis rate and should be tested in further studies.
