초록

With the advancement of medical imaging technology, the frequency of diagnostic radiological examinations such as CT and dental radiography has steadily increased, creating a need for a nationwide system that minimizes unnecessary patient exposure and ensures the safe and efficient use of radiation. However, under the current national dose management system in Korea, strategies for the systematic management and optimization of patient dose data remain insufficient. Thus, it is necessary to build a national-level infrastructure for the collection and utilization of medical radiation exposure data and to expand systems that enable structured dose management by medical institution. This study aims to expand the scope of the national dose management system to the medical radiation domain and to develop optimized management strategies. To this end, domestic and international policies and regulations on medical radiation safety management were comparatively analyzed to assess the applicability of the national system. Data were collected through quality control tests and dose management surveys of CT and dental radiographic equipment, and a framework was established for linking patient dose data with the national dose management system. Diagnostic reference levels (DRLs) were established and application strategies tailored to individual medical institutions and regions were developed. In addition, education and promotional programs were implemented to increase institutional participation and to support optimization of medical radiation use. Medical radiation safety policies in major developed countries, including the United States, Japan, and the United Kingdom, were reviewed and compared with those of Korea, and patient dose management guidelines based on International Commission on Radiological Protection (ICRP) recommendations were examined. Surveys of medical institutions were conducted to investigate the status of radiation dose management and quality control practices, and a registration and linkage framework was developed to facilitate participation of CT and dental radiographic equipment in the national system. Using registered data, data usability, system accessibility, and user satisfaction in hospitals were evaluated, and institutional improvement measures and policy recommendations were proposed. As a result, a technical and institutional foundation was established to incorporate the medical radiation sector into the national dose management system, and a standardized protocol for collecting and analyzing patient dose data was developed. Education and promotional activities enhanced institutional participation and awareness of radiation safety. The establishment of DRLs provided optimized criteria by examination type and practical guidance for exposure management. These outcomes are expected to strengthen the scientific basis of medical radiation safety policy, support data-driven decision-making, minimize patient exposure in clinical practice, and contribute to public trust through the promotion of a radiation safety culture.
