Objectives: Despite the high prevalence of dyslipidemia in patients with hypertension and diabetes, patient awareness and adherence to lifestyle modifications remain suboptimal. This study aimed to evaluate the clinical effectiveness of a structured "Intensive Care & Education" program compared to "Standard Care" within a community-based setting. Specifically, the study sought to determine whether intensive education could lead to significant improvements in lipid profiles—primarily Low-Density Lipoprotein Cholesterol (LDL-C)—and promote positive lifestyle changes among patients registered at the Hypertension and Diabetes Registration Education Center. Methods: This was a prospective, randomized, open-label, single-center clinical trial. A total of 348 eligible patients aged 30-80 years were enrolled and randomly assigned 1:1 to either the Intensive Care group (n=173) or the Standard Care group (n=175). The Standard Care group received a single session of standard hypertension and diabetes education (1 hours). In contrast, the Intensive Care group received a comprehensive intervention comprising: (1) an additional 1-hour specialized dyslipidemia education session at baseline (0 months), (2) a repeat of the same specialized education at 3 months, and (3) telephone counseling sessions at 2 and 4 months to reinforce adherence. The primary endpoint was the change in lipid profiles from baseline to the 6-month follow-up. Analysis of Covariance (ANCOVA) was utilized to assess the differences between groups, adjusting for baseline values. Results: A total of 308 patients (157 in the Intensive group and 151 in the Standard group) completed the study and were included in the final analysis. 1. Lipid Profile Improvement: The Intensive Care group demonstrated a statistically significant reduction in LDL-C levels compared to the Standard Care group (ANCOVA p=0.034). Total cholesterol levels also showed a favorable decreasing trend in the Intensive group (p=0.068). 2. Lifestyle Modification: Participants in the Intensive Care group showed a significant increase in the frequency of physical activity (days/week with ≥30 min exercise) compared to the Standard Care group (p=0.001). 3. Subgroup Analysis: While no gender-based differences were observed, the efficacy of intensive education was significantly more pronounced in patients aged <75 years (p=0.047) and those with baseline LDL-C ≥90 mg/dL (p=0.001). Conclusion: Structured and repeated intensive education for dyslipidemia significantly improves LDL-C levels and physical activity in patients with hypertension and diabetes compared to standard care alone. These findings suggest that targeting intensive educational interventions towards patients under 75 years of age and those with higher baseline LDL-C levels (≥90 mg/dL) represents a strategic and cost-effective approach for chronic disease management policy.
